Service Description
Standard check up to assess participants health and fitness as it pertains to playing a sport as outlined by the KY High School Athletic Association (KHSAA). The focus of this exam is the identification of any disease or condition that would make it unsafe to participate in a sport. Any additional testing needed may have added cost but those will be discussed up front and before they are performed.
Cancellation Policy
To cancel or change booking time please notify us 24 hours in advance. Failure to cancel an appointment within the 24 hour window will result in a $50.00 reschedule fee.
